Sixth Sense. I loved that feeling of "oh shit, he's a ghost too". You just don't get that the second time around.

Fight Club. There's another movie where one brother buys another brother an adventure - it has twist after twist. It was a very good watch, but knowing the ending ruined any chance of wanting to see it again.
Unbreakable is a movie that is only a one-time watch as well.

MFFL, that was The Game with Micheal Douglas and Sean Penn....an excellent first time watch....
Another one that comes to mind is Gladiator, I was in awe of that movie the first time I saw it.

Both of the American Pie movies. They were just so nasty that the first time you saw it, you were amazed.
Other than that, Jurassic Park. No doubt. Good LORD that movie had me in awe the first time I saw it. Also gonna have to go with Braveheart.
